To apply, for a placard and/or license plates, the Application for Persons with Disabilities Parking Placard and/or License Plate Form VTR-214 (available at www.txdmv.gov/forms) must be completed by a health care provider. The completed form can be taken to your local county tax assessor-collector’s office or mailed.
